Dexamethasone for bronchiolitis.

چکیده
To the Editor: The prospective trial of dexamethasone for bronchiolitis in infants, conducted by the Bronchiolitis Study Group of the Pediatric Emergency Care Applied Research Network (PECARN) and reported by Corneli et al. (July 26 issue),1 appears to demonstrate a lack of efficacy of oral dexamethasone in infants with a first episode of wheezing diagnosed as bronchiolitis. Although the authors aptly conclude that dexamethasone did not provide a benefit in the first 7 days after treatment, they fail to address the fact that treatment with oral steroids may prevent recurrent wheezing in the months after infection, particularly in first-time wheezing induced by rhinovirus.2 This is significant, given that rhinovirus-induced wheezing is the second most common cause of early wheezing and is an emerging risk factor for recurrent wheezing.3,4 Are there follow-up data on the condition of the children studied that can address this question?

Background and Objective: Acute bronchiolitis during infancy is the most common cause of respiratory tract infections. The purpose of this study was to evaluate the effects of a single intramuscular injection of dexamethasone on infants less than 2 years old suffering from acute bronchiolitis. Materials and Methods: In this double blind randomized clinical trial, sampling was random and from...

OBJECTIVE Determine whether dexamethasone treatment added to salbutamol reduces time to readiness for discharge in patients with bronchiolitis and possible asthma. METHODS We compared efficacy and safety of dexamethasone, 1 mg/kg, then 0.6 mg/kg for 4 more days, with placebo for acute bronchiolitis in patients with asthma risk, as determined by eczema or a family history of asthma in a first-...

OBJECTIVE Using data from the Canadian Bronchiolitis Epinephrine Steroid Trial we assessed the cost-effectiveness of treatments with epinephrine and dexamethasone for infants between 6 weeks and 12 months of age with bronchiolitis. METHODS An economic evaluation was conducted from both the societal and health care system perspectives including all costs during 22 days after enrollment. The ef...

AIM The purpose of this study was to determine the incidence of reintubation due to upper airway obstruction in a homogeneous group of ventilated infants with Respiratory Syncytial Virus bronchiolitis. Our secondary objective was to determine whether prophylactic administration of dexamethasone prior to extubation was associated with decreased risk of reintubation. METHODS This retrospective ...
